Job Summary:

 

The Direct Support Professional (DSP) plays a vital role in providing direct care, support, and assistance to individuals residing in the Home and Community Based Services (HCBS) Integrated Community Support program, formerly known as group homes. The DSP ensures the overall well-being, safety, and quality of life for program participants by promoting community integration, independence, and personal growth. This position involves working collaboratively with a team of professionals to implement individualized care plans and support individuals with diverse needs.

 

Responsibilities:



  1. Provide person-centered care and support to individuals in the HCBS Integrated Community Support program, assisting with activities of daily living, including personal hygiene, meal preparation, medication management, and household tasks.
  2. Foster a safe and supportive environment that encourages independence, community integration, and individual growth.
  3. Develop and implement individualized care plans in collaboration with the interdisciplinary team, ensuring the goals and objectives are person-centered and meaningful.
  4. Promote and facilitate community participation and socialization opportunities for individuals, including arranging and accompanying them on outings, recreational activities, and appointments.
  5. Observe and document individual progress, behaviors, and incidents accurately and in a timely manner, maintaining confidentiality and adhering to documentation guidelines.
  6. Collaborate with the behavior specialist/therapist to implement behavior support plans and strategies for individuals with challenging behaviors, ensuring consistency and positive reinforcement techniques.
  7. Assist individuals in developing and enhancing their daily living skills, such as communication, problem-solving, and decision-making.
  8. Communicate effectively and respectfully with individuals, their families, coworkers, and external stakeholders to ensure continuity of care and coordination of services.
  9. Maintain a safe and clean living environment, ensuring compliance with health, safety, and sanitation standards.
  10. Participate in team meetings, training sessions, and professional development opportunities to enhance knowledge and skills in supporting individuals with diverse needs.
  11. Adhere to agency policies, procedures, and regulations, promoting a culture of dignity, respect, and person-centered care.


Qualifications:


  1. High school diploma or equivalent (some positions may require additional education or certifications).
  2. Previous experience in a similar role or working with individuals with developmental disabilities, mental health concerns, or related populations is preferred.
  3. Demonstrated ability to provide compassionate, person-centered care and support.
  4. Strong communication and interpersonal skills, with the ability to work effectively within a team.
  5. Ability to maintain professional boundaries, confidentiality, and ethical standards.
  6. Basic computer skills and proficiency in documenting services accurately.
  7. Physical ability to assist individuals with mobility, lifting, and personal care needs as required.
