Lead Nurse

Job Summary

The Stephen Klein Wellness Center (SKWC) Lead Nurse serves as a care management leader for SKWC's intensive nurse care management program, the self-monitored blood pressure program (SMBP) while also providing clinical support for the Health Enterprise Zone (HEZ) state-funded initiative. The Lead Nurse will supervise the nurses and provide clinical insight to the Wellness Program Manager with regard to the HEZ initiative. The Lead Nurse will provide care management to targeted patients at the SKWC as well as support clinic operations as the designated clinic nurse as needed.

Essential Duties and Responsibilities

  • Provides primary supervision to the medical nurse care managers at SKWC and the self-monitored blood pressure program (SMBP)
  • Provides guidance to the Wellness Program Manager as needed on questions pertaining to the HEZ personnel, funding reports, program budget, contracts, performance measures, participant recruitment and retention, and collaboration with other health services departments.
  • Participates in meetings and planning sessions regarding the HEZ grant as necessary
  • Coordinates with SKWC site medical director on clinical staff schedules and work duties
  • Works closely with the Wellness Program Manager to ensure that nurses and community health workers collaborate to address patients' gaps in care so that nurses are able to work to the top of their licenses
  • Works closely with the Practice Manager to maximize clinical performance and efficiency.
  • Works closely with the SKWC Site Medical Director to improve provider flow, communication, and the coordination of clinically related tasks.
  • Provides oversight and management for the day to day responsibilities of the Intensive Nurse Care Management Program and the SMBP Program. Works closely with providers to determine goals, metrics, and entrance criteria of the program.
  • Provides support and education to PHHS care coordinators
  • Participates as a member of the Quality Improvement Committee and contributes to quality metrics and patient safety goals and performance. Work alongside QI committee to help PHHS meet pay for performance measures as outlined by our payers
  • Ensures that staff complete mandatory trainings and set professional development goals and plans
  • Provides performance evaluations and improvement plans for staff as required
  • Develop and maintain relevant activities to achieve Patient Centered Medical Home status.
  • Responds to any Development or Operations requests for input and information required by various funding sources.

CLINICAL

  • Provides nursing assessments and triage to walk-in patients as well as handles in-coming sick calls from patients per the Stephen Klein Wellness Center triage protocol.
  • Serves as the primary care coordinator between the patient, the patient's medical providers, behavioral health provider, pharmacist, dentist, hospital-based service providers, financial assistance specialist, residential and/or behavioral case manager, peer specialist, visiting nursing agency, community health worker and/or other specialists.
  • Demonstrates excellence in clinical care
  • Educates and counsels patients and their families about issues including, but not limited to, medications, disease and disease management, stress reduction and healthier life-style choices
  • Demonstrates excellence in care management by coordinating care with intensive emphasis during care transitions
  • Provide clinical support and supervision to the HEZ program

Education and Experience Minimum Requirements

Minimum Experience

  • Current RN license
  • CPR certification
  • 3 years of experience as Nurse Care Manager

Preferred Experience

  • Prior supervisory experience a plus
  • Two or more years' Prior experience as a Nurse Care Manager in a primary care setting.
  • BSN preferred
  • Public Health degree a plus

Core Skills

  • Knowledge and prior practice of trauma informed care
  • Proficiency in Microsoft Outlook
  • Prior experience delivering services to patients who have experienced homelessness, mental illness, and/or substance dependence, often complicated by histories of trauma.
  • Strong computer, organizational and administrative skills.
  • EHR experience

Physical requirements

Sedentary work that primarily involves sitting/standing.
