REPORTS TO: Coordinator of Children’s Case Management


POSITION OVERVIEW: The Children’s Case Manager assists children with SED and their families in obtaining necessary supports to enable their child to live at home and remain involved in the community. This is done through coordination and monitoring of services and through direct contact with the child to assist them with meeting their treatment goals.


ESSENTIAL POSITION RESPONSIBILITIES:

  1. Provides services in order to maintain required productivity/billing standard of 1175 contacts per year.
  2. Meets deadlines and ensures accuracy of various reports/paperwork, mileage sheets, and electronic timesheets.
  3. Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
  4. Submits required progress notes/billing information in a timely manner as per department, agency, MCO, and COMCARE guidelines and contracts.
  5. Participates in planning sessions with child, family, therapists and other relevant persons to develop a plan for services.
  6. Meets with child and family on regular basis, works with child to accomplish goals and meet identified needs.
  7. Monitors needs of consumer appropriately, providing level of support consistent with these needs. Notifies physician, therapist, school, and other services providers of change in status.
  8. Participates and/ or provides input for treatment planning with child and family to monitor progress.
  9. Coordinates other services required to meet identified goals.
  10. Communicates effectively with other service providers to achieve maximum care for consumers
  11. Attends school and other community meetings as determined necessary for treatment planning and coordination of services.
  12. Keeps Coordinator informed of children/youth who have been hospitalized or are at an imminent risk of hospitalization including PRTF.
  13. Informs Coordinator of any safety risks of child (SI, LEO contact, etc.)
Requirements:

POSITION REQUIREMENTS: The Children’s Case Manager must be at least 21 years of age with a minimum of three years age span between him/herself and the consumer(s). Applicants must possess a Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Computer experience required. Preferred areas include knowledge of community resources, DCF operations, courts, and school systems in accessing and coordinating appropriate services for children at risk. Effective communication skills, (oral and written), and successful experience in working with families who have children with a disability. Diplomacy, tact and collaboration building skills, and assertiveness are necessary as well as a team spirit and practice. A valid Kansas drivers license and access to personal vehicle required with insurance.


OTHER POSITION REQUIREMENTS:

  1. Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.
  2. Exhibits appropriate level of technical knowledge for the position.
  3. Produces quality of work necessary to meet job requirements.
  4. Works well with a team, treats others with respect, maintaining a spirit of cooperation.
  5. Maintains effective and professional verbal and written interactions with peers, customers, supervisors, and other staff. Demonstrates effective listening skills and is receptive to constructive feedback.
  6. Uses appropriate de-escalation and communication skills when dealing with difficult situations or people.
  7. Demonstrates the ability and willingness to handle new assignments, changes in procedures and business requirements. Identifies what needs to be done and takes appropriate action.
  8. Completes assigned work, meets deadlines without reminders/follow-up from supervisor or others.
  9. Performs work conscientiously with a high degree of accuracy.
  10. Meets goals and objectives as mutually agreed upon during last performance review (if applicable).

PHYSICAL REQUIREMENTS:

* Lifting/carrying up to 30 pounds

* Typing/data entry, writing/taking notes

* Driving (for purposes of community mobility)

* Standing or sitting for extended periods of time (during meetings and/or presentations)

* Bending/stooping/climbing stairs
