Overview

Provides speech language pathology services to patients in collaboration with the health care team consistent with VNS Home Care policies. May complete clinical co-visits for regional speech therapists annually or as required. Works under general supervision.

What You Will Do

  • Assesses the rehabilitation needs of the patient through professional knowledge and skills in observation, interviewing and comprehensive speech and language assessment.
  • Establishes and evaluates appropriate treatment plans, identifies appropriate treatment outcomes and duration of treatment, in conjunction with the physician.
  • Implements the speech language pathology plan of care by periodic assessment, planning, teaching and coordination. Recommends necessary equipment to increase client’s function and independence.
  • Manages a caseload and provides speech language pathology services to patients, as prescribed by the physician.
  • Documents all evaluation data, treatments, communication concerning patient program and discharge plans and patient’s response to therapy interventions on a timely basis. Initiates and maintains timely written and verbal communication to ensure coordinated, quality patient care.
  • Monitors the effect of speech/language pathology services on the patient/family health status and makes necessary referrals, as appropriate.
  • Appraises services provided, the need for revision of goals treatment and approaches to patient care, and discharge planning for assigned patients.
  • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
  • Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.
  • Provide care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care.
  • Transports and utilizes required technology devices (tablet), infection control materials and supplies using VNS Health designated/supplied carrying case (backpack) weighing approximately 15 – 20 lbs to and from patient homes/care facilities, VNS Health offices and therapists’ home
  • May provides clinical co-visits for agency Speech Language Pathologist staff; co-visits may occur in geographical areas outside of assigned region. May provide clinical case review for SLP staff as requested by Rehab Supervisors.
  • Acts as a resource for education and consultation with clients, families and Agency staff and documents education and consultation activities. Participates in initiatives, projects, work groups and programs representing Speech Therapy services, as requested.
  • Participates in Agency in-service programs, QA review, and interdisciplinary team meetings as requested, and consults with team members, as necessary.
  • Adheres to Agency policies and procedures.
  • Assumes responsibility for continued professional growth.
  • Participates in special projects and performs other duties as assigned.

Qualifications
  • License and current registration to practice as a Speech Language Pathologist/Audiologist in New York State required.
  • Valid driver's license may be required, as determined by operational/regional needs.
  • Certificate of clinical competence to practice speech therapy from a NYS-approved program required.
  • Master's Degree in Speech Language Pathologist or equivalent required.
  • Minimum of one year experience as a Speech Language Pathologist preferred.
  • Bilingual skills may be required, as determined by operational needs.
  • Basic PC skills preferred.
