Responsibilities $5,000 sign-on bonus available for new hires, relocation assistance also available for those that qualify. Charter House is a continuing care retirement community with four different care units. Our Charter House team members are expected to cross train to work in our various care units. Home health services are provided from 7 am – 11 pm in the independent living resident’s apartment. Our Assisted Living Center provides 24/7 care for residents who need minimum to moderate assistance and transfer assistance of one person. Our Supportive Living Center provides 24/7 care to residents requiring moderate to maximum assistance including two-person transfer assist, more dementia friendly programing, and assistance with dining. Our Short-Term Rehab Center is our skilled nursing facility and primarily serves as a transition for care between the hospital and return to home. Qualifications Graduate of an Accreditation Commission for Education in Nursing (ACEN) accredited associate's degree or diploma program. If graduated from a nursing program that was not accredited by ACEN, at least one year of RN experience (within the last five years) in an applicable care setting is required. If graduation did not occur within the last two years, one year of RN experience in an applicable care setting or one year of current LPN experience at Mayo Clinic is required. One year of work experience in Long Term Care or Home Health or two years of acute care nursing is preferred. Additional Qualifications Must be able to work independently. Requires good communication and human relation skills to interact with Health Service and Charter House staff. Strong problem solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process for groups of resident and employees. Must be able to adapt to unpredictable situations and stress. Works flexible hours, which may include days, evenings, nights, holidays and weekends. License or Certification MN Registered Nurse license; Basic Cardiac Life Support Certification for Health Care Providers (must come from one of the following providers: American Heart Association, Red Cross, or Military Training Network).
