Connections at Home VIA Willow Valley Communities is seeking an on call as needed in home Caregiver. The in home Caregiver will provide companionship, light housekeeping services and personal care to individual clients in their own homes.
Responsibilities:
- Following an Individual Care Plan developed for the client by Connections at Home.
- Communicating to supervisor any changes or concerns regarding a client's health and/or the appropriateness of services being delivered.
- Document in software system any change of condition or anything out of the ordinary and/or communicate to your supervisor.
- Ability to use the software system or app to review client's care plan; to review personal calendar and schedule.; and to document any change of client's condition.
