Job Description
Ohio's Hospice LifeCare, located in Wooster/New Philadelphia is looking for a full-time Team Physician to join our mission! The Team Physician will serve under the direction of the Medical Director and work primarily in an inpatient setting at the Stanley and Flo Gault Inpatient Pavilion, in addition to covering homecare teams.

The Team Physician provides expertise in caring for patients with life limiting, serious illness and their families. Responsible for assisting with the provision and maintenance of medical direction to hospice/palliative care teams which includes consultation, review of patient care, staff education, and liaison activities within the medical community. The overall goal is to improve quality of life for every patient and family we have the privilege of serving.

Qualifications:

  • Active Unrestricted Ohio license to practice Osteopathic or Allopathic Medicine.
  • Board certified or eligible in Hospice and Palliative Medicine preferred.
  • Unrestricted DEA license.

Key Responsibilities:

  • Provides medical direction and support to hospice staff to ensure continuity of care and compliance regulations.
  • Participates in the development of individual plans of care.
  • Schedules and performs face to face assessments of hospice patients to estimate prognosis and evaluate for continued hospice eligibility.
  • Participates in decisions related to admissions, discharges, level of care changes and care planning for hospice patients to provide seamless care to patients and ensure the highest quality care.
  • Performs patient assessments and other medical procedures, including the prescribing of medications consistent with the collaborative practice agreement and conforming to state and federal law always.
  • Provides emotional support to patients and families utilizing other Interdisciplinary Team (IDT) members as needed.
  • Ensures that the appropriate level of care – home care, respite, continuous care, or general inpatient - is utilized for hospice patients to achieve maximum palliation of symptoms and optimal use of the IDT.
