Join Our Team! HFA Home Visiting Case Managers Needed

  • Have the ability to travel within the Greater Philadelphia region for home visits and outreach?
  • Possess strong interpersonal skills for effective collaboration with diverse groups?
  • Have a desire to play a pivotal role in delivering home-based services that empower pregnant women and parenting families with children 0-3?

Position Summary

The HFA Case Manager (Advocate) provides in-person, family-centered, evidence-based health education, parenting education, child development education and activities. Case management services consist of completing assessments, facilitating referrals and any other beneficial supportive services.

Essential Functions:

  • Provide intensive case management services, including assessments and referrals, through in-person home visits in one of the following services areas: Berks County, Bucks County, Delaware County, Philadelphia County, Lehigh County or Montgomery County
  • Flexibility is key - be ready to adapt your schedule to meet family needs.
  • Assist families in identifying their health and social service goals.
  • Support parent-child engagement and foster family well-being through reflective strategies.
  • Collaborate with community resources to ensure comprehensive support for families.
  • Conduct health promotion activities and deliver evidence-based parenting education.

To be well-equipped and excel as an HFA Case Manager you will possess the below qualifications:

  • Associate's degree in Social Work, Public Health, or related field of study such as: Family Studies, Women’s/Gender Studies, Human Services and Education.
  • Experience with community health issues.
  • Valid driver's license with driving experience and a clean driving record.
