Overview of Position: Under administrative supervision of the Clinic Manager / Office Manager/ Practice Administrator, the Staff Nurse will be responsible for coordination of care between clients, medical providers and the infectious disease specialist. The Staff Nurse is primarily responsible for the support and logistical functions necessary for the effective and efficient delivery of health care services in the clinical setting

Qualifications: Diploma in nursing with 2-years’ experience; or AD in nursing with 1-year experience; BS in Nursing with 1-year experience preferred; Licensure through State Board of Nursing of South Carolina. Current SC nursing license and CPR certification.

Knowledge/Skills: Maintain current general medical knowledge. Provides the clients with consistent and reliable medical information and shares with family as deemed appropriate by the client. Prior to performing job related activities in this position one must be familiar with information including basic critical care and chronic illnesses management and willingness to learn about AIDS Service Organizations and Community Health Centers. Understand company and program policies and procedures as well as be aware of HopeHealth’s mission, vision and values. Should be knowledgeable and possess counseling skills in prevention interventions. The staff nurse should possess the ability to utilize basic software applications to include Microsoft Office, electronic medical records (EMR) such as (e-Clinical Works (eCW) and Provide Enterprise (PE),if position requires). Ability to communicate effectively utilizing both oral and written means. Must possess a current driver’s license, be dependable, have reliable transportation and mode of communication (mobile or home telephone) with messaging capability. Willingness to be flexible and ability to work with clients and physicians from a variety of backgrounds and lifestyles while maintaining non-judgmental attitude. The staff nurse must be a team player and able to multi-task. Additionally, possess the ability to respond appropriately to emergencies and work in a high-pressure environment. Ability to take initiative and handle various tasks simultaneously while working efficiently, effectively, and independently. Knowledge of adult learning principles, management leadership principles, and community resources. Knowledge of policies, standards, regulations, and protocols.

Essential Job Functions:

· Assists medical providers in the evaluation and treatment of clients to include new patient intakes, chart preparation, taking and recording of vital signs, obtaining lab work, communicating with pharmacists and other physicians’ office personnel, managing/maintaining records, and providing appropriate education and counseling to clients regarding their specific health care needs.

· Assesses health, nursing and personal care needs of individuals and their families.

· Assists with medication refills, medical record requests, and medication guarantees.

· Assisting with procedures such as pap smears and breast exams and completes the plan of care after medical visit including referrals, if necessary.

· Facilitate clinic flow and room turnover.

· Provides hands on care during clinic- administers injections/ vaccinations, provide wound care, set- up pill boxes; assist with lab draws as needed; triage and

monitoring of vital signs and client status; educate clients and family on medications, treatments and basic disease processes.

· Keep work areas neat, tidy and well stocked and supplied; Stock clinic rooms with supplies.

· Perform data entry tasks for reporting purposes.

· Participation in training opportunities to enhance knowledge such as conferences and continuing education/in-service events.

· Responds to phone calls in a respectful and timely manner.

· Responsible for medication log sample med list sign out; Responsible for labeling sample meds.

· Filing of labs into medical record.

· Assists with the development and promotion of relationships in the service area with medical providers.

· Communicates and works with medical providers and other agencies in planning and implementing care of clients, their families, and other contacts.

· Participates in Continuous Quality Improvement (CQI) activities, to include making recommendations for strategies to improve quality of care.

· Provides direct care to clients in busy, fast-paced clinical setting with multiple providers and non-clinical staff.

· Other duties as assigned.

Physical Requirements: Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
