Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
  • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.

Migrant Health Outreach Worker

$2500 Sign on Bonus!!

Proficiency in Spanish and English required.

Location: Benkelman, McCook Nebraska and Surrounding Area.

Please reside in Benkelman, McCook NE. or within the surrounding area.

  • 50% home office environment with clients and with the public
  • 30% outreach in the community and clients
  • 20% training, meetings, and community work with other organizations

Full-Time, Monday-Friday, 8AM to 5PM, may require occasional weekend, All Major Holidays Off

Areas of Responsibility: Generally working from a home office, promote access of migrant and seasonal farm workers to Migrant Health services and assist with clinic flow.

Major Duties:

  1. Positively promote the Migrant Health Program and facilitate accessibility for migrant and seasonal farm workers which may include transportation, to provide interpretation and translation services.
  2. Work cooperatively with other agencies, programs and become knowledgeable regarding other services provided and to act as resource for clients.
  3. Schedule appointments with providers and make referrals to other services.
  4. Provide case management to chronic patients.
  5. Follow up on missed appointments and reschedule appointments if needed.
  6. Provide health education related programs and materials to clients.
  7. Enter client data into Electronic Medical Records (EMR), assisting in maintaining client tracking system, maintaining all required documentation in client files such as attaching medical, labs, radiology, basic intake forms and proof of income into EMR.
  8. Accurately enter data in Microsoft Word and Excel to update spreadsheets, keep patient’s information current and up-to-date in their charts such as patient names, DOB, current phone numbers, current mailing/physical addresses.
  9. Entering new and re-enrolled patient’s information into the EMR.
  10. Always maintain client confidentiality.
  11. Provide general clerical duties input data, scanning documents, answering calls, returning calls in a timely manner, laminating, mailing/emailing applications and completing statistical data records.
  12. Maintenance and care of company vehicle which may be provided. This vehicle will be strictly used for Migrant Health purposes only. Care includes interior and exterior cleanliness. Report any needed maintenance or damage to vehicle.
  13. Track mileage on the CAPWN milage form.
  14. Responsible for safely managing company credit card for fuel purchases.
  15. Receive prior approval from the Migrant Health Manager for any other credit card purchases, completing a Credit Card Report Form and emailing to the Migrant Health Manager prior to the card being used.

Working Conditions:

  • Work requires meeting tight deadlines.
  • Significant travel in and around the region transporting clients and family members.

Physical Requirements:

  • Requires long periods sitting, driving and computer data entry.
  • Fine manipulation of hands with repetitive action to operate office equipment.
  • Occasional lifting and carrying up to 20 pounds.
  • Must regularly talk, hear, and have close vision ability.

Pay Grade contingent upon experience $14.96 - $17.50.

Minimum Qualifications:

Grade: 12

  1. High school diploma or GED.
  2. Must be proficient in communicating in Spanish and English.
  3. Age 21 or older.
  4. Must have a valid driver’s license with good driving record and insured vehicle (a company vehicle may be provided.)
  5. Physically and mentally able to perform duties.
  6. Available to work irregular hours and weekends.

Grade: 13

1. Meet the minimum qualifications above.

2. Two years’ experience in outreach and client referral or related area.

Grade 14

  1. Meet the minimum qualifications above.
  2. AA degree in Human Services, Social Work, or related field and one year experience in a human service program or five years’ experience in in outreach and client referral or related area.
