CARE COORDINATOR SPECIALIST

Adagio Health Inc is looking to hire one (1) full-time Care Navigator for its Seneca, PA office.

The Care Coordinator Specialist plays a crucial role in facilitating access to healthcare services for our clients. This position involves working closely with individuals to assess their healthcare needs, developing personalized care plans, and guiding them through the healthcare system to ensure they receive the necessary services.

RESPONSIBILITIES

• Client Assessment and Support: Conduct thorough assessments of client needs, focusing on behavioral health, food insecurity, transportation, housing, and other essential community services.
• Knowledge Development: Gain in-depth knowledge of available community support services and programs, including the referral process for each.
• Veteran-Specific Expertise: Become adept in military benefits, documentation, and health insurance navigation to support our veteran community effectively.
• Referral Management: Provide well-coordinated referrals, both internally and externally, ensuring a seamless transition for clients.
• Barrier Identification and Resolution: Assist clients in identifying and overcoming barriers to service access, utilizing local resources and supports to optimize care.
• Educational Support: Offer education and resources to clients as needed, enhancing their understanding and engagement in their own care.
• Documentation and Data Management: Meticulously document all client interactions and data, participating in data analysis and reporting.
• Quality Improvement Participation: Engage in initiatives aimed at enhancing the quality of services provided.
• Collaboration and Outreach: Work closely with clinical and program staff, maintaining open communication lines. Conduct outreach to build and maintain relationships with community partners.
• Veteran Mental Health First Aid (MHFA): Obtain and provide Veteran MHFA to partners and veteran audiences.
• Follow-Up: Ensure continuous care by providing consistent follow-ups with patients, clients, and partners.
• Teamwork: Collaborate with various community service providers, including clinical, nutrition, and ancillary support services.
• Technology Utilization: Employ virtual/tele-health and texting program equipment for remote client interactions.
• Additional Responsibilities: Be adaptable to perform other duties as assigned.

QUALIFICATIONS FOR A CARE NAVIGATOR

• Bachelor's degree from an accredited university in Social Work, Public Health, or a related field
• Preferred experience in:
  • Case management/care planning
  • Customer service
  • Enrollment processes for medical insurance products
• Proficiency in Microsoft Office products
• Excellent communication skills
• Ability to undertake some regional travel.
