Case Managers provide case management services.  Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive needs through communication and resources to promote well-being. 

 

Qualifications:

  1. Bachelor of Arts or Bachelor of Science degree in Counseling, Social Work or related field preferred. Degree may be waived with equivalent experience.
  2. Knowledge of community resources preferred.
  3. Bilingual required.
  4. Must be 21 years old or over.
  5. A valid California driver’s license with less than two minor moving violations is required of staff who have driving duties to meet the requirements of their position and be approved by the agency’s insurance company.

 

Essential Job Functions:

  1. Ability to communicate ideas in oral and written form.
  2. Computer skills including Microsoft Office.
  3. Ability to instruct in group and individual settings.
  4. Physical, mental, and emotional health adequate to perform duties.
  5. Ability to work effectively with cultural/ethnic diversity.
  6. Adhere to all applicable legal and ethical standards.
  7. Ability to work at various locations, including client homes, schools, and community settings.
  8. Sound personal qualifications (i.e., maturity, stability, high moral standards, and sound mental health and physical ability to perform job duties.)

 

Responsibilities:

  1. Provide access, advocacy, resources, and linkage to clients.
  2. Interact productively within a multi-disciplinary team and other community organizations.
  3. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
  4. Attend community events.
  5. Report any suspected child abuse immediately to direct supervisor or utilize the chain of command if supervisor is not available. 
  6. Report any suspected adult/elder abuse immediately to your direct supervisor or utilize the chain of command if supervisor is not available. 
  7. Report any imminent danger to self or danger to others to direct supervisor or utilize the chain of command if supervisor is not available. 
  8. Attend staff meetings, designated meetings, and trainings.
  9. Cross training to support various roles within programs as needed.
  10. Responsible for preventing and controlling infection.
  11. Responsible for maintaining a culture of quality and safety.
  12. Effectively use de-escalation skills and techniques for crisis prevention.
  13. Properly administer the C-SSRS to help prevent suicide.

 

Other:

  1. Attend all required meetings.
  2. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  3. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  4. Other duties as assigned.
