BH CARE COORDINATOR

Central Virginia Health Services seeks a full-time Licensed Bachelor of Social Work (LBSW) to provide coordination of behavioral health and case management and consultation for patients in an integrated primary care medical setting. Services to be provided to a diverse socially vulnerable patient population with complex health needs. Social worker must be able to effectively work as a member of a multidisciplinary team, provide some brief behavioral interventions under supervision, and collaborate with community resources to assist patients. Social worker must be comfortable documenting all services in the electronic medical record in a timely manner. Competitive benefits available.
