Case Manager
Express Employment is searching for a Case Manager who will have responsibilities that include advocating for and assisting in connecting individuals with health services, housing, and social services. Position is full time, long term!
Schedule: Monday - Friday 8AM - 4:30PM w/occasional OT
Location: Sacramento
Pay: $25.00 -$30.00/hour
Case Manager highlights:
- Provide support, mentorship, and education for individuals facing health challenges.
- Conduct telephonic and in-person outreach to Medi-Cal beneficiaries and vulnerable community members.
- Collaborate with agency partners to conduct community outreach.
- Assist in obtaining health coverage and housing readiness documentation.
- Work in various environments, including shelters, home visits, and community clinics.
- Connect clients with appropriate housing services and support.
- Maintain client confidentiality and adhere to confidentiality requirements.
- Establish positive relationships with mental health programs, shelter programs, and local service providers.
- Attend team meetings, case conferences, and community meetings as needed.
- Maintain accurate client records and activity logs.
What we look for in a Case Manager:
- High school diploma or GED required; Bachelor’s or Master’s degree in social work or related field preferred.
- Experience working with individuals experiencing homelessness or complex health issues desirable.
- Certification as a Drug & Alcohol Counselor (CDAC) preferred.
- Awareness of socio-cultural characteristics of the population served.
- Cultural awareness and competency skills.
- Strong interpersonal skills and ability to connect with diverse populations.
- Detail-oriented with excellent time management skills.
- Ability to work flexible hours, including evenings and weekends.
- Valid driver’s license and reliable transportation required.
- Ability to pass background/fingerprint check.
