DESCRIPTION OF WORK

The care manager is responsible for handling various case assignments related to general casework, intake, and assessment. You will work closely with older adults, starting from the initial contact with individuals, their families, and professionals, and continue to provide ongoing care management responsibilities. In addition to this, you will also assist older adults and their families in completing program forms and accessing services. The role involves specialized duties and requires maintaining a caseload of consumers. The care manager regularly coordinates services with physicians, community agencies, financial institutions, hospitals, nursing homes, personal care homes, etc. You are also responsible for responding to client emergencies and ensuring protective services are in place when needed. The care manager is a multitasking professional who pays meticulous attention to detail. This position will handle various tasks, including paperwork completion, data input of confidential client information into state databases, and meeting deadlines. The role requires efficient time management and organizational skills to ensure all work assignments are completed accurately and on time. The care manager receives training and is supported by a team to enhance their skills and knowledge in the field.

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours/week
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Periodic on-call hours when required, upon successful completion of training.
  • Out of county overnight travel may be required.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy will be given to candidates who live within Schuylkill County. If no eligible candidates who live within Schuylkill County apply for this position, candidates who reside in other counties may be considered.
