Star Nursing has Urgent Needs for local LVN’s in Fortuna, CA!

Facility: Skilled Nursing 

Location:  Fortuna, CA  

Start Date:  ASAP  

Shifts: AM and PM shift

Length: 13-week contract

**Each facility will only accept 13-week contracts, and will not approve time-off requests**

Pay Rate:   

Local LVN: $43/hr. 

LVN Travel No Housing: $20/hr. taxable + $1000 per week / $25/hr. non-taxable

Responsibilities and Duties

·                     Provides for activities of daily living by assisting with serving meals, feeding patients as necessary, ambulating, turning, and positioning patients; and providing fresh water and nourishment between meals.

·                     Maintains patient stability by checking vital signs and weight, testing urine, and recording intake and output information.

·                     Provides patient comfort by utilizing resources and materials; transporting patients; answering patients’ call lights and requests; and reporting observations of the patient to the nursing supervisor.

·                     Documents actions by completing forms, reports, logs, and records.

·                     Maintains work operations by following the policies and procedures of the facility

·                     Protects the organization’s value by keeping patient information confidential.

·                     Enhances nursing department and facility reputation by accepting ownership for accomplishing new and different requests; and exploring opportunities to add value to job accomplishments.

·                     Checks residents/patients frequently to ensure that their personal care needs are being met.

·                     Treat all residents, visitors, and staff with courtesy

·                     Maintaining professionalism at all times

Requirements: 

Step # 1 

·                     Resume - long-term care experience 

·                     Nursing license – LVN 

·                     Current - BLS/CPR 

·                     Skills checklist 

·                     1 to 2– current -Supervisor references 

Step # 2 

·                     Physical (within the last 12 months) 

·                     PPD (within the last 12 months) 

·                     MMR, Varicella, 

·                     Hep B (or decline) 

·                     Tdap/Flu (or decline) 

·                     Exams 

·                     LGBT Training Certification 

·                     Copy or driver’s license and SS card 

·                     Must be able to pass background/ urine drug screen
