Our care coordinators assist clients in improving their lives through a better understanding of how to manage their behavioral health needs, physical health needs, and other psychosocial factors that impact their recovery. Duties include, but are not limited to:

  • Completing psychosocial assessments
  • Strengths-based treatment planning
  • Care coordination with other providers
  • Health promotion and wellness
  • Client advocacy
  • Client and family support
  • Referral to additional community and social support services
  • Small group facilitation

General Responsibilities and Professional Expectations

  • Demonstrates a commitment to ongoing improvement by actively engaging in supervisory sessions, and setting and achieving personal and professional goals annually.
  • Supports departmental objectives under RBA (Results Based Accountability) by:
  • Identifying and understanding client needs.
  • Aligning work with adopted standards to ensure client well-being.
  • Collecting and analyzing data to support performance measures.
  • Making data-driven recommendations to enhance client satisfaction.
  • Developing and maintaining strong internal and external community relationships.
  • Ensures personal safety by recognizing workplace hazards, adhering to ergonomic methods, complying with agency safety policies, reporting potential risks, and promptly addressing work-related injuries.
  • Actively support the agency's mission and values, contributing to a positive organizational culture.
  • Works collaboratively with colleagues within and across departments by maintaining a solution-oriented attitude, respecting competing demands, and striving for the best outcomes for the agency.
  • Builds effective relationships with a diverse range of professionals and community resources, including but not limited to:
  • Health care providers (primary care, therapists, psychiatrists, medical specialists).
  • Community support services (food pantries, soup kitchens, shelters, general assistance offices).
  • Government and social services (DHHS, Social Security, housing authorities).
  • Housing sector (landlords, housing support services).
  • Substance recovery and mental health support facilities.
  • Financial management services (payee services).
  • Advocates for clients' needs and rights, navigating various systems to access necessary resources and services.
  • Maintains accurate and timely records of client interactions, treatments, and outcomes, ensuring compliance with legal and ethical standards.
  • Remains vigilant and prepared to effectively handle and de-escalate crises, providing immediate support and intervention as needed.
  • Demonstrates a deep understanding and respect for diversity in all its dimensions, including cultural, racial, gender identity, socioeconomic, and ability differences. This commitment ensures inclusive and equitable service delivery, acknowledging and valuing the unique experiences and perspectives of all AWS clients.

The sign-on bonus is available in two installments. The employee must be in good standing to obtain the bonus.



  • Current MHRT-C or MHRT-C provisional OR eligible for a MHRT-C/p (paid MHRT-C/p training for the right candidate)
  • Ability to think independently and make decisions
  • Compliance with agency, state, and federal guidelines
  • Driving is an essential part of your position and responsibilities
  • Must pass a background check and not be on the state or federal suspension and debarment list
  • Must pass criminal and CPS background check
  • Physical Demands: Typing for long periods with proper breaks, lifting up to 20 lbs, walking up stairs, and standing for long periods of time.
  • Working Conditions: The position is in the office and community.


Compensation details: 21-25 Hourly Wage
