Salary:

PneumaCare Health and Wellness is dedicated to serving the most vulnerable members of our community by fostering stability and resilience in the areas of Community Supports, Enhanced Care Management and Day Habilitation.  We are supported by the mission of the Ministerial Association of Colusa County (MACC), our mother organization, by partnering with the local church to empower our communities through the Gospel.


Under the direct supervision of the CalAIM Program Manager, the Outreach Coordinator is responsible for member outreach engagement.  Oversees and implements the Member Information File (MIF).  Coordinates with Community Care Navigators and Supervisors to ensure referrals are input and maintained in an accurate and efficient manner.


Duties and Responsibilities:

-Engages eligible members, offers intake services where the member lives.

-Input members’ records into Electronic Health Record.

-Submit accurate and timely referrals to member’s health care provider for Community Supports, Enhanced Care Management and Day Habilitation services.

-Maintain health provider portals for accuracy regularly.

-Provide a variety of outreach attempts to members who qualify for services and document attempts.

-Provide necessary documentation for regular reporting needs to the appropriate supervisor.

-Works in conjunction with a care team to provide tailored and timely services to members.

-Attend required training.  Facilitates internal training/updates to clinical or administrative staff members.

-Collaborates with Program Manager to ensure Outreach templates and procedures are appropriate and effective at capturing needed data/information.

-Reviews existing MACC protocols and polices to match requirements.  Develop protocols, policies, and accompanying workflows.

-Coordinate and organize organization’s involvement in community events.

-Coordinate, provide immediate response and communicate response to received referrals from members, potential members, or community members/organizations.

-Follow HIPAA guidelines and requirements during all interactions with members, community members and in requesting required medical records

-Uses motivational interviewing, trauma-informed care, and harm -reduction approaches to engage with members and community organizations.

-Other duties as assigned.



Knowledge, Skills, and Abilities:

-Strong documentation skills.

-Ability to multi-task and prioritize when needed.

-Ability to independently seek out resources and work collaboratively.

-Ability to use computers and learn new software programs.

-Ability to develop and maintain good working relationships with staff.

-Excellent interpersonal skills reflecting clarity, diplomacy, and the ability to communicate accurately and effectively with all levels of staff and management.

-Demonstrates ability to work in a regulatory climate that includes oversight and deferral entities, payer contracts, etc.

-Possess ability to communicate effectively, both verbally and in writing.

-Proficient knowledge of Microsoft Outlook, MS Word, and Excel.

-Able to travel and attend professional meetings, conferences, trainings, and sites.


Requirements:

-At least 2 years of case management or related experience in the field.

-Office management experience, social services case management, experience providing services to low-income populations are a must.

-Candidates with less experience will be considered if lived experience is indicated.
