POSITION SUMMARY:

The Program Manager has responsibility for overseeing the day-to-day operations of multiple sponsored homes and providing support coordination services for the residents and families. This position is responsible for providing supervisory oversight of program, facilities, and independent contractors in accordance with agency policies and procedures, licensing standards, and accreditation standards. Support coordination responsibilities include assuring employment/day support for residents, establishing community linkages and assuring success for adults who are developmentally disabled in partnership with families, guardians, advocates, professionals, churches, and public and private agencies.

DUTIES AND RESPONSIBILITIES:

  • Provide services and oversight for multiple sponsored homes contracting with HumanKind.
  • Possess depth and maturity to work closely with a challenging population, which does not have adequate public or private services to meet quality of life demands.
  • Possess the ability to work independently.
  • Provide a team model of service delivery by effectively supervising and empowering direct care staff.
  • Maintain the established budget and objectives with required reporting.
  • Individual Support Plan (ISP) development and implementation in the sponsored homes.
  • Oversight of the residential home including the training, social, spiritual and moral development of the residents.
  • Act as a liaison between families, employers, and other community resources.
  • Maintain quality documentation of the quality of care each resident receives.
  • Ensure that the host families in each sponsor home maintain current trainings and certifications as required.
  • Travel within the community as needed.
  • Deliver person centered residential support services, especially as it related to sponsored residential homes.
  • Perform job duties in accordance with licensing and accreditation standards.
  • Have 24h-hour on-call availability.
  • Work under the supervision of the Sponsored Homes State Manager.

KNOWLEDGE AND ABILITIES:

  • Effective communication with adults with developmental disabilities, their families, and other service providers; counseling techniques, individual support plan development and implementation, writing goals and objectives for residents and documentation techniques.
  • Effectively communicate written and verbally to a diverse range of people. To include adults with developmental disabilities, their families, contractors, and other service providers.
  • Knowledge of best practices and techniques of residential programs for adults with developmental disabilities and support coordination services.
  • Working knowledge of DBHDS licensing and human rights regulations and accreditation standards, supervisory techniques, effective skill building for adults with developmental disabilities across the life span, team work and motivational theory.
  • Understanding of Home and Community Based Services (HCBS) Settings Rule which includes the rights of individuals receiving services, and required efforts to create ‘a life like ours’.
  • Understanding budgeting and statistical reporting, and state funding for adults with developmental disabilities.
  • Follow complex oral and written directions, plan and carry out all documentation for the day-to-day operations of the home.
  • Demonstration of time management skills required to perform inspections of all sponsor homes and create documentation in a timely manner.

MINIMUM QUALIFICATIONS:

  • Bachelor’s Degree in Social Work, Psychology or related field.
  • Some experience working in the field of developmental disabilities.
  • Meets Federal requirements to be a Qualified Developmental Disability Professional (QDDP).
  • Experience in the operation of sponsored placement residential services is preferred.
  • Provide “TB FREE” documentation.
  • Valid Driver's License and acceptable driving record.
  • Complete all required background checks and pass drug test.
