Direct Support Worker (DSW) For Fredonia KS area
Summary
Work for persons with disabilities in the Fredonia KS area who are self-directing their care, assisting with activities and tasks according to their Plan of Care (may involve: activities of daily living, personal care, meal preparation/housekeeping in individual’s home). And may involve assisting person with grocery shopping, going to doctor appointments, and other errands as needed.

Part-time / Full-time
Essential Job Functions
- Provide assistance with activities and tasks that are unique to a person’s disability needs in accordance to a Plan of Care
- Promote activities and work on goals that will help a person remain independent and in their home
- Provide transportation as needed
- Assist individual with appointments and activities in the community
- Maintain a safe and clean environment in the person’s home
- Stay in contact with SKIL Fiscal Agent (providing payroll services) regarding required documentation, such as hire packets, activity logs, time worked, etc.
Job Requirements
- At least 18 years of age
- Have a valid Driver's License
- Have previous experience working in direct care
- Clear background checks
