Job Description
Mountain Region Home Health is currently seeking a Full or Part-Time Home Health Physical Therapist to assist our patients in Lee County, Virginia.
Responsibilities:
- Conduct comprehensive assessments to identify the individual needs and goals of patients.
- Develop customized treatment plans tailored to each patient's unique condition and goals.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Collaborate with the interdisciplinary healthcare team to ensure coordinated care and the best possible patient experience.
- Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans.
- Educate patients and their families on effective techniques for maximizing independence and managing their conditions.
- Adhere to all ethical and professional standards, as well as state and federal regulations.
Current PT license required
One year of Home Health experience preferred.
