Job Summary:


The purpose of this role is to oversee all aspects of ongoing client care, including: conducting initial client assessments; overseeing completeness and accuracy of Care Plans, Client Service Agreements, and pricing for new and existing clients; verifying that the Caregivers assigned to each client are an ideal fit based on skills and additional match factors; managing ongoing care management, client relationships, and client satisfaction; conducting supervisory visits and Care Plan updates at least every 90 days; coordinating with partner organizations to provide holistic care and wellness support for each client; and supporting in the development of improved caregiver training to ensure delivery of best-in-class care.

The ideal candidate for this role has experience in social work, geriatric care management, or a related field. You must be an excellent communicator, adept at navigating complex client care situations to make sound recommendations, must be organized and efficient, must be customer service focused and able to foster relationships with clients, fellow PHC employees, and community partners, must be technically competent and willing to learn our web-based scheduling system (Wellsky/ClearCare), and must thrive on helping families navigate challenging situations.

The goal is to ensure that our clients care needs are always met with a comprehensive and individualized Care Plan, a team of the most qualified and best-fit caregivers, and that our clients and team members are treated professionally at all times.

Applicants should ideally have past experience in, or familiarity with non-medical home care.


Responsibilities:
  • Managing client care for all current clients, conducting initial in-home assessments for all new clients to develop Care Plans, conducting reassessment visits for each client at least once every 90-days to ensure Care Plan, pricing, and Client Service Agreements are up-to-date and aligned with evolving client care needs.
  • Conducting introductions (meet-and-greets) of new caregivers to clients and families to ensure a smooth start of care and a strong caregiver-client match.
  • Managing Quality Assurance program, including using Net Promoter Score to monitor client and family member (Account Owner) satisfaction.
  • Ensuring that caregivers assigned to each client are an ideal fit based on skills and additional match factors, working closely with our Employee Care Manager to make recommendations and adjustments as needed.
  • Supporting community marketing efforts to establish trusted referral relationships with local organizations.
  • Supporting the development/certification of caregiver training and advancement programs to ensure best practices are implemented and proper care techniques are being applied.
  • Reporting weekly on performance, KPIs, and progress towards goals.
  • Serving as backup On-Call Scheduling Coordinator in scheduled rotation with office team, supporting primary On-Call Coordinators with all off-hours phone calls and restaffing of shift call-offs.
Qualifications:
  • Academic background and experience as Geriatric Care Manager, gerontologist, social worker, RN, or other related field required
  • Outstanding skills in customer service, leadership, decision-making, verbal and written communications, and organization
  • Alignment with PHC Core Values and Mission
  • Impeccable character, strong interpersonal skills, high emotional intelligence, and resilient under stress
  • Strong sales skills, goal-oriented, and receptive to feedback and coaching
  • Skilled at using WellSky software program to manage client care and schedules
  • Strong technical skills, attention to detail, and ability to quickly learn new technologies and procedures
  • Ability to be clear, fair, professional, and consistent in all communications
  • Work well and make sound decisions in time-sensitive situations
  • Reliable with respect to confidentiality
  • Experience in private duty home care or related field a plus
  • Age 21 or older
  • Ability to pass a comprehensive background and reference check
  • Must have reliable vehicle, insurance, and be willing to drive to clients across Greater St. Louis area
