JOB TITLE: Activity Director


REPORTS TO: Registered Nurse, Licensed Practical Nurse/ Department Manager


JOB SUMMARY:  The Activity Director  is a professional caregiver who provides direct and indirect care activities  under direct supervision.


JOB RESPONSIBILITIES: Responsibilities include but not limited to:

  • Prepare unit/patient room for receiving a patient/resident for admission
  • Provide personal care assistance including: bathing, hygiene, grooming, dressing, eating, toileting, transferring, and ambulating
  • Obtain and document vital signs, weight and intake/output as directed
  • Document accurately daily activities and pertinent information
  • Make regular rounds to assigned patients, anticipate their needs, and answer and respond to call lights in a timely manner
  • Transport patient/resident as necessary, including utilization of transport vehicles.
  • Provide post mortem care when necessary
  • Work with other team members to turn and transfer patients as directed
  • Will plan, prepare, and offer activities for all skilled and intermediate swing bed patients. 
  • Will document activities offered, participation, and alternate activities offered as appropriate in EHR.
  • Will complete activity assessment on admission and with changes of level of care with all skilled and intermediate swing bed patients.
  • Will have an updated activity assessment with resident’s periodic comprehensive review.
  • Will participate in a patient's care plan meeting.
  • Will plan and post an activity calendar.
  • Will encourage patients to attend activities.  If a patient refuses the majority of group activities, the director will plan and help implement one-on-one activities to meet patient needs and choices.
  • Coordinates operation of Resident and Food Council meetings. 
  • Participates in Quality Improvement projects as directed.
  • Will complete a restorative care plan in conjunction with the rehab department to inform rehab of any noted changes in physical or mental abilities.  Will document accordingly.


SKILLS AND QUALIFICATIONS: 

Skills- Ability to multitask, Excellent Time Management, Critical Thinking, Adaptable to ongoing changes in healthcare, Able to identify and follow protocols


  • Experience
  • Prefer but not require a minimum of one year clinical experience.
  • Education
    • High School graduate (or equivalent) or high school student with an interest in the healthcare profession.
  • Registration/Certification
    • Current CNA licensure in the state of Kansas.
    • Activity Director certification preferred but not required.
    • CPR
