DESCRIPTION

Decypher is seeking a Clinical Pharmacist candidate to support the Naval Medical Center San Diego, CA.

RESPONSIBILITIES

  • Develop and document therapeutic plans, evaluate medication therapy
  • Order, review, and analyze laboratory test and other diagnostic studies necessary to monitor and support the patient’s medication therapy
  • Prescribe medications, devices, and supplies to include: initiation, continuation, discontinuation, monitoring, and altering therapy based on established formulary protocols and in accordance with P&T approved scope of practices
  • Order consults, as appropriate, to maximize drug therapy outcomes and disease state management
  • Monitor the patient’s response to drug therapy through all available sources, such as, patient a pharmacy records, and laboratory data
  • Educate and counsel the patient regarding his/her drug therapy and/or protocol

QUALIFICATIONS

  • Bachelor’s Degree or Higher in Pharmacy
  • Pharmacy License
  • 2 year of experience as a Clinical Pharmacist performing direct patient care clinical activities in an inpatient, outpatient, or ambulatory care pharmacy

  • Basic Life Support (BLS)
