Job Description

Our team of direct placement recruiters are seeking compassionate guides to help support patients & families through in-home, end-of-life care through hospice.

 

Shift/Hours:

Monday-Friday, 8:00 AM - 5:00 PM

 

Holiday/Weekend Requirement:

Weekends - Occasional, dependent on area needs

 

On-Call Requirement/Travel Requirement:

Approximately 1 overnight shift per week, every 4th-5th weekend (overnight) depending on location and census. Candidates will travel to various locations from the base office and will be reimbursed 58 cents per mile.

 

Patient Care:

1. Perform all duties and responsibilities in accordance with the Nurse Practice Act and basic principles and guidelines of professional nursing.

2. Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs, including a complete physical assessment of all current and previous health history.

3. Evaluate patient status by:

a. Writing and initiating a plan of care

b. Regularly re-evaluating patient and family/caregiver needs

c. Participating in the revision of the plan of care as necessary

4. Use health assessment data to determine nursing diagnosis.

5. Develop a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Include patients and families in the planning process.

6. Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed and determined by the physician’s plan of care.

 

Additional Duties:

1. Participate in on-call duties as defined by the on-call policy.

2. Attend interdisciplinary group meetings.

3. Supervise ancillary personnel and delegate responsibilities when required.

4. Assume responsibility for personal growth and development. Maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

5. Actively participate in quality assessment and performance improvement teams and activities as assigned.

 

Must-Haves:

- License: RN license of the state they are applying to

- Certification: AHA approved BLS/CPR certification

- Experience: One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care preferred.

- Education: Associate's Degree

 

Preferred:

- Bachelor's degree
