Since 2012, Vermont Support & Stabilization (VTSS) has been dedicated to empowering Vermont's youth and families. We're expanding our team and seeking enthusiastic individuals to provide dynamic individual and family therapy to at-risk and adjudicated youth in the Central/Southeastern region, encompassing Barre, Montpelier, Springfield, and surrounding counties in Vermont.

As a Permanency Coordinator with VTSS, you'll play a pivotal role in offering short-term support and stabilization services. Your mission will be to guide clients and their families in identifying and leveraging long-term community support and resources. We're seeking candidates with post-training experience in therapy, eligibility for licensure, and a background in working with traumatized children and/or adolescents.

This full-time position involves traveling an average of 8+ hours weekly throughout Vermont to family's home, with evening shift flexibility required. Responsibilities include assisting youth and family overcome obstacles, coordination of treatment, discharge planning, case management, community integration, documentation, and active participation in weekly team meetings.

Summary:
We are looking for a team player within our community-based service model, Vermont Support & Stabilization (VTSS). This position is being sought in our Central area covering the area from Barre to Hartford, VT. VTSS's Clinical Services are intended to provide short-term support and stabilization. As such, services provided by the Permanency Coordinator should generally be focused on helping clients and their families identify and utilize long-term community support, natural support systems, as well as agencies in the community. The right candidate will have experience working with traumatized children and/or adolescents and a career focus in human services/social work, therapeutic services, and/or clinical services.

Permanency Coordinator Duties:
  • Assist youth and families to overcome obstacles, identify strengths, and develop a support network to ensure they overcome their challenges and achieve success.
  • Coordinate treatment and discharge planning to achieve goals of the permanency plan.
  • Support, educate, and guide in developing structure, routines, effective parenting skills, positive communication, safety, behavioral plans and hygiene plans.
  • Interact with the community through court and in community-based meetings with service providers, schools, Designated Agencies, and DCF.
  • Other services provided are based on individual and family needs (i.e., nutrition and meal planning, household budgeting, etc.).
  • Attend weekly supervision and participate in team meetings and case reviews.
Education Requirements:
  • Preferred: Bachelor's degree with a focus in human services/social work or therapeutic services.
  • Minimum: Associate's degree with a focus in human services/social work or therapeutic services.
Other Requirements:
  • Valid driver's license with currently active car insurance.
  • Satisfactory completion of background, driving, and applicable licensing checks.
