Job Description

We are looking for a Full-Time Registered Nurse (RN) Case Manager in Suffolk County.  This role will cover an assigned territory, it's a field-based RN position requiring someone to drive and cover an assigned territory. You will be responsible for educating patients and their families on prevention and healthy habits. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. The goal is to promote patients’ well-being by providing high-quality nursing care.

Responsibilities:

  • Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.
  • Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.
  • Develop the Plan of Care in coordination with all health care professionals assigned to the case.
  • Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges.
  • Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
  • Report to the Clinical Manager on a daily basis.
  • Provide documentation using the provided iPad within the required time frame.


Qualifications:

  • Licensed Registered Nurse (RN) in the state of New York with at least 1 year of clinical experience; Home care preferred
  • COVID Vaccinated
  • Oasis experience is strongly desired
  • Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
  • Have reliable transportation and be willing to commute to provide the best in-home patient care.
  • Willing to take advantage of our excellent training system comprised of in-house and field orientation and become an elite healthcare provider!
  • Some weekend days are required, and summer and winter holiday commitments as needed.
