Position Summary:
As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice. Perform administrative duties such as completing medical forms, reports, evaluations, and charting as required. Ensures that nursing personnel are providing direct care to residents and providing such care in accordance with the residents' care plan and wishes.
Reports to: Director of Nursing
FLSA: Non-exempt
Minimum Qualifications:
x High school diploma or equivalent preferred
x Associate’s degree preferred
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Certifications:
x Basic Life Support (BLS) or ACLS supersedes the BLS requirement
AND First Aid OR First Aid CPR AED within 30 days of hire
x Food Handlers Certification within 30 days of hire
XFingerprint Clearance Card underway on hire, Card must be turned in within 60 days after hire
Licenses: Arizona State LPN Licensure or compact state required on hire
