Family Educator/Home Visitor/Case Manager

The Champaign-Urbana Public Health District (CUPHD) is hiring a full-time Family Educator/Home Visitor/Case Manager position with their GREAT Start Prevention Initiative Birth to Three Home Visiting Program. The GREAT Start program provides free and voluntary home visiting services and support to families in Champaign County with children under the age of three, and prenatal families. The selected candidate will perform professional work in the Baby TALK Model of family engagement.

Responsibilities include:

  • Conducting child developmental assessments.
  • Facilitating developmental activities with children and caregivers, developing family goal plans, and providing case management and connection to community resources.
  • Program goals include supporting the child's development through strengthening the parent-child relationship, establishing and deepening working relationships with families, empowering parents to set goals for themselves and their children, and supporting families' mastery in their growth and development.

Requirements & Qualifications:

  • Graduation from an accredited college or university with a bachelor's degree in sociology, social work, psychology, human services, child development or a closely related field,
  • or two (2) years of professional social work, counseling, or closely related experience
  • A valid driver's license, dependable transportation, and proof of auto insurance.
  • Spanish speaking individuals are highly desirable.
  • Experience working with parents and children under three years of age is highly preferred.

The starting annual salary for a Case Manager is $50,515 for a 40-hour week.
