POSITION TITLE: CLIENT SERVICES SPECIALIST (Non-Medical Case Manager)

REPORTS TO: Clinical Supervisor

SUPERVISES: N/A


**This position does offer a hybrid work schedule, where some work is done remotely from home, some in office, and some out in the community.

**This position does not require a social work license.



POSITION SUMMARY:

This position provides intervention and case management services for HIV+ individuals and their family members. Client Service Specialists must adhere to standards of care as set by Caracole, provide HIV-related teaching, facilitate access to case management, medical care, medication access, oral health care, psychosocial support and referrals for a broad range of services.


RESPONSIBILITIES:

  • Provide case management intervention services, including initial assessment of service needs, development of a comprehensive individualized service plan, continuous client monitoring to assess the efficacy of the plan, and re-evaluation of the plan as required by funding bodies.
  • Effectively utilize bio-psychosocial models as related to HIV care and continuous assessment of client and/or family members' needs and personal support systems.
  • Ensure access to high quality HIV medical care by providing assistance in obtaining and maintaining health insurance, benefits/entitlement programs, and other sources of support for covering costs of medications and medical care.
  • Provide education regarding safer sex practices, medication adherence and the importance of linkage to medical care.
  • Serve as a liaison between clients and medical/service providers.
  • Participate in individual/group supervision meetings.
  • Maintain current, comprehensive knowledge about entitlement programs and community resources.
  • Act as a resource and support to staff and clients concerning knowledge and application to benefit/entitlement programs.
  • Participate in quality management activities.
  • Maintain client and statistical records as required by regulatory or funding agencies, and Caracole.
  • Meet all required deadlines for documentation.
  • Maintain a collaborative working relationship with Caracole team members.
  • Identify and pursue educational opportunities to promote professional growth.
  • Represent Caracole in the community.
  • Adhere to all rules, policies and regulations pertaining to privacy and confidentiality.
  • Provide transportation when needed.
  • Other duties assigned.


QUALIFICATIONS:

Bachelor's degree in social work or related field with at least six months experience in a social service setting, or equivalent experience. Knowledge about HIV/AIDS service networks, entitlement programs and other community services. Proven ability to work harmoniously with ethnically, culturally, and racially diverse groups of individuals.

REQUIRED SKILLS:

  • Ability to maintain confidentiality in verbal, print, and electronic communications.
  • Basic computer skills, including competency with Microsoft Office.
  • Read, write, speak and understand English with excellent verbal and written communication skills.
  • Sensitivity to persons living with chronic disease (HIV/AIDS and/or addiction) and co-existing social, mental and physical conditions.
  • Excellent attention to detail.
  • Bilingual (Sp/En) preferred.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

This hybrid, hourly position includes time spent working in office, remote at home, and in the community. Work is generally 9-5 M-F. The employee is frequently required to sit, stand, walk, talk, hear; use hands and fingers, handle or operate objects, operate a telephone, fax machine and computer. The employee is frequently required to move throughout the building and to other sites in the community. Some site visits may involve stairs. Employee must have reliable transportation and ability to drive to other sites and meetings as needed. Must have driver's license, insurance and vehicle in order to transport clients. The employee must be able to lift and/or move up to 25 pounds. COVID 19 vaccination (or waiver) required upon hire.


SALARY AND BENEFITS
