A DAY IN THE LIFE OF A HOME CARE CLINICAL MANAGER

The Clinical Manager is responsible for clinical oversight, supervision and expertise to Home Care, Hospice, MCH/CIS and High Tech program staff and for providing goal directed nursing services to a wide range of patients. Responsibilities include but are not limited to the following:

Key responsibilities include (but are not limited to):

  • Actively oversees the integration of new and existing patient schedules to maximize staff productivity
  • Actively evaluates staff productivity levels and takes action to ensure that staff meets Agency productivity level
  • Proactively promotes case management patient care model of care. Takes action to promote effective and timely communication between team members and across disciplines/programs
  • Provides clinical oversight to RN's, LPN's to ensure care plans and associated work
  • Proactively develops orientation agendas, including but not limited to, documentation, preceptor pairings, on-line training, staff development schedules, etc..
  • Proactively fosters and maintains a positive environment of change and accountability amongst staff to meet changing agency needs. Analyzes a wide range of operational data and produces reports, including, but not limited to, productivity, time and travel trends, visit trends, etc. Takes action and initiative to ensure that information is accurate. Makes recommendations to Home Care Director on findings in a timely manner
  • Proactively addresses patient complaints as they arise and takes action to promptly address issues in a constructive and effective manner

Qualifications:

  • Degree in nursing, physical, occupational, or speech therapy required
  • Current Vermont State license in either RN/PT/OT/SLP required
  • Valid Driver's License

Experience:

  • Five years of clinical experience required. Home Health experience highly desired. Two years in a supervisory position required.
