Join our New Hampshire Detox Team as a Case Manager, in Bethlehem, NH, powered by TruHealing. We are seeking a Case Manager to work alongside the clinical team and ensure life-skills improvement and long-term sobriety for patients with SUD and Mental Health Diagnoses.

***$1,000 Sign On Bonus***


*Sunday-Thursday 8am-430pm**


We Require:

  • HS Diploma required, some college preferred
  • 1+ years of experience working in the behavioral health field

Expectations of the Role:

Your job begins when a client is admitted to our facility.

  • Upon admission, conduct a case management interview with patient to determine if there is a need for FMLA, Short Term Disability Insurance and if there are any pending legal issues. Will also obtain ROI for referent, family, EAP and/or employer. Ensure patient has notified family of arrival.
  • Produce all documentation needed for the patient's employer, schools, legal, etc. including, but limited to, FMLA, STD, LTD, Treatment Letters, etc.
  • Meet with each patient once weekly, at a minimum, for case management appointments.
  • Develop a strong, ongoing relationship with patients and their families. Maintain ongoing healthy communication with the patient's families.
  • Develop effective aftercare plans for all patients. Ensure that all patients follow their establish aftercare plan. Work with the outreach team or company representative attached to the patient to determine an appropriate aftercare plan.
  • Update and modify ALL the patient's discharge dates, which are determined by the Utilization Review, Clinical, and Medical team.
  • Accurately share information in team meetings.
  • Ensure medical records are sent to the facility where a patient is discharging to.
  • Facilitate SUD and Mental Health groups that are assigned.
  • Maintain a basic understanding of health insurance benefits, health insurance terms, and other payment methods that are used for our organization.
  • Remind patients of the program rules and regulations as necessary and report infractions appropriately.
  • Interact appropriately with patients, showing them respect while maintaining boundaries.
  • Protect the privacy of all patient information in accordance with the organization's privacy policies, procedures and practices, as required by federal and state law, and in accordance with general principle of professionalism as a health care provider.
