Salary:

POSITION: Case Manager  / Care Coordinator                                                              


RESPONSIBILITIES:

  1. Provide care coordination/case management and linkages with services such as, but not limited to: transportation including to health care or dental appointments, prescription assistance, eligibility assistance with enrolment in entitlement/mainstream benefits, translation and other identified needs to improve access to and utilization of care. 
  1. Assist patients with completing required paperwork, explain clinic policies and processes to help patients understand and navigate available services
  2. Assist uninsured patients with identifying and determining a health care payor source (e.g. insurance, Medicaid, Medicare, VA or other public benefits) as eligible and/or access to community-based resources (e.g. medication assistance programs).
  1. Assist with service delivery to patients and their families utilizing trauma-informed and evidence-based approaches. 
  2. Assist with health education/disease prevention by linking patients to community resources and/or providing information where gaps exist. 
  1. Maintain records and reports using integrated health records which can be audited for accountability.
  2. Responsible to maintain, and possibly establish if not present, an outside network of applicable community resources, contacts, and maintain relationships as seen fit for CM.
  1. All other duties as assigned 

 

MINIMUM QUALIFICATIONS: Preferred graduate of an accredited college or university with a Bachelor's degree in a related human service or health care field; must have a valid Kentucky driver's license and must pass background check.


SPECIAL KNOWLEDGE, SKILLS AND ABILITIES: Prefer a minimum of two years' experience working with and sensitivity for the homeless/low income population and persons with behavioral health care disorders and knowledge of the Appalachian culture and have knowledge of available community resources.
