Job Summary: Provides direct and indirect patient care in the ambulatory care setting. Provides care that meets the psycho-social, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Care Manager. Maintains regulatory requirements, nursing and ambulatory care policies, procedures and standards. Communicates with physicians and team members about changes in patient's clinical condition, including results of diagnostic studies and symptomatology.

Demonstrates Competency in the Following Areas:

  • Provides direct patient care under supervision, evaluates outcomes, consults with other healthcare team members as required and adjusts nursing care processes as indicated to ensure optimal patient care.
  • Ability to perform a head-to-toe assessment, under supervision, on all patients and reassessments as per policy. This includes neonatal, pediatric, adolescent and geriatric patients and the general patient population.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient's age.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Communicates appropriately and clearly to team leader, coworkers and providers.
  • Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skills.
  • Demonstrates the ability to perform treatments and provide services to level of licensure.
  • Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with ambulatory care standards.
  • Performs other duties as assigned within the LPN scope of practice.
Requirements:

Regulatory Requirements:

  • Current LPN license in good standing for the State of Louisiana
  • Current BLS certification

Required Skills:

  • Able to communicate effectively in English, both verbally and in writing
  • Proficiency in Spanish desired
  • Basic computer knowledge desired
  • Experience with an EMR system preferred
