This full-time position involves collaborating to provide management and delivery of patient care plans in Scranton, PA and surrounding areas. The role requires an associate degree or higher from an accredited School of Nursing, RN license in the state of work, and at least two years of RN experience, with hospice experience preferred. The successful candidate will have the ability to spend approximately 20% of their day driving to/from patient locations and demonstrate proficiency with tablets, mobile phones, and EMR software.
