Description

Lutheran Social Services is currently seeking a Care Coordinator with a substance use recovery focus in a new program serving Sauk County. The Care Coordinator will work with Sauk County residents who are incarcerated to prepare for release from county jail or state prison and smoothly transition back to the community with the appropriate support services. . This is a Full Time position and benefits eligible.

The Care Coordinator will be housed at the county jail to facilitate access to inmates. They will complete a risk assessment at booking (at the jail or court) and identify potential eligibility for intervention programs. The availability of re-entry services will be discussed with incarcerated individuals if they meet criteria. The Coordinator will monitor the jail population and identify Sauk County residents who will be incarcerated a minimum of 30 days and are scheduled to release within three months. As this is a new position and program, the role will also require some program development of tasks and responsibilities in collaboration with program leadership.

Under direction, performs work involving the assessment, service planning, and implementation of the service plan for assigned individuals. Performs other duties as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.

  • Provides screening and assessment for clients in a locked, restrictive setting who could benefit from non-narcotic, non-addictive, injectable medication assisted treatment, with supervision
  • Completes level of care assessments (ASAM), substance abuse assessments, diagnostics, screening, and referral. Obtains required additional supervision and signatures when using UPC, ASAM, SUD assessments such as SUDDs or DSM-5/ICD 10 criteria.
  • Prepares and completes all required documentation according to state, federal and agency standards in a timely manner.
  • Coordinates, plans, and facilitate administration of NNAI
  • Offers client sessions, including group, family, and individual services.
  • Provides person-centered and trauma informed case management and release planning services and learns to clients with attempts to connect family and individual supports
  • Coordinate the various social, community and mental health and/or substance abuse services received by clients
  • Assists clients with navigating insurance benefits and eliminating barriers to services
  • Attends and participates in staff development programs, including in-services training, staff meetings, clinical supervision meetings, and similar opportunities.
  • Utilizes supervision to provide services in a timely manner according to program, licensing and contractual requirements.
  • Provides communication internally and to our external partners, both written and verbal, with supervisory oversight
  • Participate in program development, grant reporting, and evaluation including Quality Assurance review and initiatives
  • Establishes effective public and working relationships and collateral contracts.
  • Serves as a positive role-model for clients and their families.
  • Performs other duties as required.

EDUCATION AND/OR EXPERIENCE:

  • Bachelor's degree in human services or a related field
  • A minimum of 1 year of experience working with a correctional population preferred or required, depending on contract requirements.
  • Working knowledge of addictions and recovery, counseling and case management principles and methods.
  • Ability to learn and retain information regarding the federal, state, and local rules and regulations governing the delivery of SUD services.
  • Ability to provide addictions services to clients and their families.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • The candidate must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
