Prism Health is the only nonprofit LGBTQIA+ health center providing comprehensive primary care, behavioral healthcare, and pharmacy services in the Pacific Northwest (Oregon, Washington, and Idaho). Since opening its doors in 2017, Prism Health has become a model for culturally specific care and now serves over 1,500 patients with a wide range of services, from day-to-day primary care and pharmacy needs to targeted services like gender-affirming care and specialized population health screenings.

CAP is excited to announce that we are hiring an experienced Registered Nurse I. The Registered Nurse (RN) I is an integral part of a multidisciplinary team that provides comprehensive healthcare to patients at Prism Health. The RN works autonomously and in collaboration with the clinical and integrated behavioral health team to provide comprehensive patient care, health education, and disease prevention using a trauma-informed approach and evidence-based approach.

The RN also plays a key role in the Quality Improvement metrics throughout the clinic and collaborates with other members of the Quality Improvement Committee to improve and implement clinic workflows and ensure compliance with Federally Qualified Health Center (FQHC) policies, procedures, and quality assurance standards.

Travel throughout the county and evening and weekend work are occasionally required.

ESSENTIAL FUNCTIONS

Provide comprehensive nursing care to all ages in the FQHC setting.
Work in collaboration with other members of the healthcare team to provide trauma-informed, patient-centered, and evidence-based care.
Conduct comprehensive assessments of patients including physical exams, vital signs, phone triage, medication reconciliation, and social needs assessments.
Administer medications, vaccines, and other treatments as prescribed by providers or in accordance with existing standing order protocols.
Perform nursing visits based on nurse visit protocols.
Perform triage, both in person and over the phone.
Perform short-term case management and care coordination in collaboration with other members of the primary care and integrated behavioral health team.
Facilitate report to emergency departments or direct patient admission to the hospital when needed.
Provide education to patients including but not limited to disease management, health coaching, preventative care, and medication instructions.
Document patient encounters accurately and in a timely manner within the electronic health record.
Adhere to Federal HIPAA rules and regulations.
Participate in quality improvement initiatives and evidence-based practice projects.
Provide support and guidance to medical assistants and other clinic staff as needed.
Other duties deemed necessary to achieve the mission of the organization.

NOTE: This is a non-management, union-represented position. The compensation, benefits, and conditions of work for this position are collectively bargained.

COMPENSATION: $89,523.20 annually

Successful candidates will have:

3+ years nursing experience
Bachelor’s degree in nursing required.
Basic Life Support card
Valid nursing license
Advanced knowledge of EMRs
Willingness to work a flexible schedule and to fill in when needed.
Excellent bedside manner and communication skills
Ability to explain treatments and conditions to patients of all ages and diverse populations.
Working knowledge of health care laws and regulations
Successful experience working with ethnic, racial, economic, and sexually diverse populations and strong understanding of issues relevant to PLWHA, gay/bisexual/queer men, transgender individuals, the larger LBGTQ+ community, and how those issues affect or influence service delivery.
Demonstrated commitment to advancing equity and inclusion in workplace or community settings.
Demonstrated ability to effectively collaborate with colleagues in a professional environment to best serve the needs of a client/patient.
Excellent written and oral communication skills at all levels and professional relationships, including interpersonal solid communication ability.
Excellent organizational and time management skills, with strong attention to detail
Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet.
Ability to work independently with accountability and exercise sound judgment, discretion, and professionalism at all times.

MINIMUM QUALIFICATIONS:

Minimum 1 year of nursing experience
Bachelor of Science in Nursing (BSN)
Basic Life Support certification
Valid Oregon and Washington RN license

PREFERRED QUALIFICATIONS:

Experience working within an FQHC or a community health setting.
Experience providing healthcare using a trauma-informed and harm reduction lens.
Lived experience of systemic oppression and non-dominant cultures. From Black, Indigenous, and other people of color; transgender, non-binary, and other LGBTQ+ people; and people with disabilities.
Demonstrated commitment to advancing equity and inclusion in workplace or community settings.
Knowledge of community resources in the Portland metro, including Multnomah, Washington, Clackamas counties.
Experience with phone triage, preferably experience using Schmitt-Thompson Protocols.
Excellent organizational and time management skills, with strong attention to detail.
