OVERVIEW: Founded in 1979 in direct response to rising rates of hunger and homelessness in North San Diego County, Interfaith Community Services works to break the cycle of poverty and homelessness by Helping People Help Themselves. Interfaith is the most comprehensive social services agency in North County, providing both immediate safety net services and long-term, life-changing programs designed to meet the holistic needs of people in crisis. Together, in partnership with 250+ diverse faith communities, more than 650 recurrent volunteers, and a growing staff of over 200, Interfaith provides a safe haven and pathway towards self-sufficiency for more than 17,000 community members annually.


WHAT YOU WILL DO: This position will be specifically assigned to Interfaith’s California Advancing and Innovating MediCal (CalAIM) Recuperative Care Program (medical respite care). Case Managers are responsible for providing intensive case management to individuals who are experiencing high medical, psychiatric, and substance use disorder recovery needs and are experiencing homelessness or with unstable living situations that are too ill or frail to recover from an illness (physical or behavioral health) or injury in their usual living environment; but are not otherwise ill enough to be in a hospital. Post-discharge treatment in a medical respite setting allows individuals to achieve or maintain medical stability and prevent hospital admission or readmission and achieve a more adaptive level of psychosocial functioning while in the program.

  • Provides comprehensive case management to individuals who do not have a residence and who have high medical or behavioral health needs
  • Conducts intake, bio-psychosocial needs assessment, housing assessment
  • Develops and implements collaborative care management and housing support plans which support individuals’ needs in the areas of physical health, mental health, substance use disorders, community-based services support, social supports, and social determinants of health
  • Assists individuals in accessing additional benefits and related documentation such as, Social Security Insurance (SSI), CalFresh, cash aid, and obtaining required documentation (ID, birth certificate, social security card, financial records, etc.)
  • Conducts discharge planning to appropriate community housing including application assistance, linkage to resources and referrals to facilitate independence and transition to stable housing
  • Collaborates and communicates effectively with an interdisciplinary and external care team
  • Coordinates care with managed care plans (MCP), community providers, other referral sources and caregiver/authorized support person(s)
  • Provides crisis intervention as part of team
  • Participates in individual and group case consultation
  • Completes services and reports as assigned by Interfaith and CalAIM MCP, both clinical and administrative within the allocated time frame
  • Maintains up to date documentation for services provided to respective individuals
  • Maintains a flexible schedule according to client needs
  • Participates in all staff meetings and trainings
  • Performs additional duties as assigned

* Case managers may be assigned to work with Veteran or non-Veteran

Requirements:

EXPERIENCE / EDUCATION

  • Master’s Degree in Social Work (MSW), Master’s Degree with an emphasis in Marriage and Family Therapy (MFT) or related field required
  • License-eligible, and currently registered and in good standing with the State of California Board of Behavioral Sciences as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT) or related field preferred
  • Two (2) or more years of relevant work experience required
  • Knowledge of substance use, mental health, assessments, intervention and coordinated care strategies in a community non-profit setting preferred
