JOB SUMMARY

Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospice. Provides professional, comprehensive, family-oriented services to individuals in the hospital setting. Assesses patients' and families' psychosocial, environmental, and financial needs. Formulates, implements goals of care in collaboration with patient, family, and other caregivers, and provides case management as appropriate. Offers counseling services to provide information and support during all facets of serious illness to assist the patient/family. Coordinates with community, homecare and hospice agencies as appropriate..

REQUIREMENTS

• Required Master's Degree Successful completion and graduation from an accredited school of social work, with a Masters Degree in Social Work.

• 2-4 years Requires minimum of three years of clinical experience; one year in a community health setting preferred. Required

• LCSW - Certified Social Worker

• LCPC - Licensed Clinical Professional Counselor

SPECIFIC REQUIREMENTS

Interpersonal communication (both oral and written) required in order to educate and comfort patients and their families; maintain effective contacts. Maintain appropriate documentation records. Professional licensure required in Maryland
