PSRI - Case Manager

Location: Dublin, GA; Emerging Adults Program

Job Description/Duties:

• Ensure individuals on case load are linked with services needed in the community. Record effort in PSRI GIRPP note (Psychosocial Rehabilitation Individual) within 2 ours of service being provided.
• Ensure individuals go to scheduled doctors' appointments.
• Ensure individuals take medication prescribed.
• Will complete at least 65% productivity per day working with individuals on case load.
• Complete progress notes no later than 24 hours after delivering the service.
• Will not leave Emerging Adults unattended during hours of operation.
• Document the individuals' degree of daily participation, response, behavior, and intervention provided in GIRPP progress note.
• Will obey all safety rules while operating CSB vehicles and complete all routes in a timely manner.
• Transport Emerging Adults to and from program outings as needed.
• Communicate accurate information to others in professional and courteous manner.
• Identify team goals and ways to work with coworkers to accomplish those goals, work to keep group activities productive.

Job Requirements/Preferences:

• Minimum 1 year of relevant experience.
• Preferred bachelor's degree in human service-related field.
• Valid GA Driver's License.
• Must be 21+ to be able to drive company vehicles.
• Basic Computer Skills required.
