RESPONSIBILITIES:

As a Registered Nurse Assessment Coordinator (RNAC), your primary responsibility is determining the appropriate level of care for our residents.
Scheduling and overseeing all resident MDS Assessments.
Participating in the pre-admission process to determine potential residents’ level of care.
Reviewing all MDS assessments and related forms to certify timely completion.
Analyzing Quality Indicator reports for potential resident concerns and problems.
Participate in the Quality Assessment and Assurance Program.
Understand and follow Resident Care Plans.
Ensure accurate documentation during your shift.
Complete accurate charting, reporting, and progress notes.

REQUIREMENTS:

Possess an active and good-standing RN License.
Long term care experience preferred.
Experience as an RNAC preferred.
