REGISTERED NURSE 1(RN) PSYCH – Terra Alta, WV
WISE MEDICAL STAFFING IS NOW HIRING 1 REGISTERED NURSE’S (RN) FOR CONTRACTS IN (THE STATE YOU ARE STAFFING)
(6) month contract and up to 40 hours per week. Overtime is possible but is not guaranteed and can be mandated.
PAY RANGE: Gross up to $1920 weekly
JOB REQUIREMENTS:
  1. Resume
  2. Two References
  3. APS/CPS Authorization and Release for Protective Service Record Check
  4. WV Cares Self Disclosure and Consent Form (2 pages)
  5. WV License Verification
  6. WV Cares Clearance Letters
  7. Background Check
  8. WV OT Form
  9. 10 Panel Drug Test (Current)
  10. CPR Card Approved by Red Cross or American Heart Association.
  11. Physical
  12. TB (PPD) Test (2)
  13. Influenza Shots (seasonal)
  14. Hepatitis B Declination
  15. Copy Driver’s License/ SS Card
  16. COVID-19 Test—Negative within 7 days of reporting
  17. If fully vaccinated, we need a copy of the COVID-19 vaccination card.
If not vaccinated, must have COVID-19 Exemption form completed and submitted to our HR Department for review
