Wraparound Facilitator

KVC West Virginia is one of leading companies within the child care system, specializing in areas such as in-home care, that include wraparound services and CSED waiver. We are currently looking to hire someone in Barbour County area. If you enjoy working within the community and serving children in need we are the company that can do that. This is hybrid job position where you will be working with clients within their homes as well as being provided a flexible schedule through working from your own home. Your home office will be our Martinsburg office.

Service Area to include Mercer County and surrounding areas

Minimum Requirements:

Education; Bachelors Degree from an accredited college or university in a social service related field.

Licensure/Certification License by the West Virginia Board of Social Work Examiners or equivalent board preferred but not required

Experience: Minimum two years experience in children and family services, with preferable knowledge and demonstrated experience of Wrapround and trauma. A Masters Degree in related Social Services field may suffice for lack of experience.

Position Summary: The Wraparound Facilitator will be responsible for direct service coordination and wraparound service implementation in the facilitator role. This staff will work in both Safe at Home, CSED, and other wraparound programs as dictated by state program.

Essential Duties and Responsibilities include the following. Other duties may be assigned according to the qualification of staff and development of programs.

• Will work to build a child welfare system that is safe, secure, and stable; family centered, connected to the community and which relies on evidence based practice necessary to support services.
• Will work with family to define and engage Formal and Natural supports.
• Collaborate with community partners and agencies to ensure resources are being utilized effectively efficiently.
• Will work with all parties involved to ensure that timelines for Engagement and Planning implementation Maintenance, Transition and Aftercare are met and that services are provided effectively and within Wraparound principles.
• Provide services to biological family members such as parenting visitation and transportation
• Works with the family through each phase in wraparound process, with a frequency, intensity, and duration of service provision defined by the Wraparound Team and adjusted as dictated throughout each phase.
• Will make required face to face home visits to the foster/adoptive and biological home a minimum of one time per week and more often as prescribed by the treatment plan.
• Will hold at least one family team meeting per month
• Will attend all Multi-Disciplinary Team (MDT) meetings. Will also attend all IEP meetings
• Will travel (possibly to multiple counties in a service area) and be able to work in homes of families that are served
• Will be willing to travel to Residential Facilities (both in state and out of state) who are serving youth that are transition home form the out of home care system
• Will be knowledgeable in the areas of child development parenting skills, mental health, wraparound and trauma.
• Will have knowledge of de-escalation techniques.
• Ensures that all aftercare visits are scheduled, attended and outcomes are reported to Wraparound Supervisor
• Will work with community members involved in the child's life including the birth family and other natural supports in which the family relies.
• Will have a good working relationship with the Department of Health and Human Resources (DHHR)
• Meets with Wraparound supervisor for a minimum of one hour bi-weekly to provide supervision
• Participate in administrative supervision as requested
• Responsible for meeting performance expectations including contract requirements, timelines, regulations and laws.
• Provides direct services per program, state, and contract guidelines.
• Ensures services are in compliance with all regulatory standards and contract terms and conditions.
• Complete all required paperwork and follows agency policies and procedures within timelines, to ensure smooth paper flow
• Attends and participates in all scheduled meetings, court hearings, consultations, and staff meetings as required
• Acquires and develops resources to meet identified needs of clients and staff
• Provides positive feedback to clients, families, community team members and staff
• Provides culturally relevant and sensitive services to clients and ensures staff do the same.
• Provides back up support to contract services as requested
• Observes children for signs of abuse and/or neglect
• Maintains a working relationships and communications with DHHR and other community partners
• Understands contract requirements and work to improve contract specific outcomes
• Attend meetings seminars and other educations functions as identified by the supervisor or agency director
• Complete ADP Core Competencies for Wraparound Facilitators and annual courses according to agency schedule.
• Maintain valid driver's license and auto insurance.
