Mental Health Case Manager- Polk County, TN

What will I be doing?

-In this position you will help ensure that client’s are consistently receiving medical care, behavioral health services, and access available resources within their communities.

-Contact clients via telehealth 2-4 times monthly to ensure quality and efficacy of services rendered.

-Will report regularly to leadership to receive support and guidance in providing needed services.

Qualifications:

-Bachelor’s degree required with preference to human service fields or other health related discipline. Experience with case management and work with children and families preferred.
