Responsibilities/Accountabilities We have an amazing opportunity for a Case Manager that can: Acts as facility liaison with county social workers, community services boards, Medicaid, SSI, Social Security, legal aide, and other services Performs assessments of residents Experience/Skills/Education Required: Bachelor's degree (B.S.) in Social Work or related field (i.e., Sociology, Psychology, Nursing) from a four-year college or university with experience in a related field and a minimum of five years of related experience and/or training or equivalent combination of education and experience.
