Summary*
This position is responsible for delivering evidence-based wellness promotion and self-care management solutions to all AllCare Medicaid, Medicare, and dual eligible pediatric members and their families/guardians to identify social determinants of health barriers, assist navigating the community-based network of services, provide benefit education, facilitate communication between the member and their providers, and assist in member development of a self management plan. Additionally this position serves as a lead for the maternal child health team.
*Essential Duties*

* Assessment, planning, implementing, coordinating and requesting services required to meet member’s health care needs to ensure timely access to medical, behavioral health, community services, and to ensure that all services are provided within HIPAA guidelines.
* Collaborates with the care coordination supervisor to organize, facilitate, and oversee the maternal child health team.
* Represents the health plan in community activities that promote the health plan’s community service partnerships.
* Acts as an advocate for members in problem resolution and facilitates collaboration with the interdisciplinary care team (ICT) in order to assist members to develop a knowledge base that will allow self-reliance.
* Conducts thorough planning to determine and document specific objectives, goals, and actions to meet the member’s identified needs, including re-evaluation of the individualized care plan (ICP).

Supervisory Responsibilities*
This position will act as a lead for the Maternal Child Health Team and works closely with the Care Coordination Supervisor to organize, facilitate, and oversee the Maternal Child Health Team.

Education & Experience*

* An Associate’s degree or Bachelor's degree from an accredited college or university. BSN is preferred
* At least one year experience working in a Pediatric setting
* Working knowledge of the Oregon Health Plan with special attention to the rules and policies affecting pediatric, obstetrical, and members with disabilities
* Leadership experience preferred

Certificates, Licenses, and/or Registrations*

* Valid, unencumbered RN license in the state of Oregon
* Valid insurable driver’s license
* Current vehicle insurance
* Current BLS certification
