About this job

APRN will work in skilled nursing facilities and participate in weekly bedside wound rounds with nursing home staff, change dressings and perform medically necessary debridement, establish plan of care and work within facility protocols. Wound Care Certification required.

Schedule: Monday - Friday (8am-5pm)


JOB SUMMARY

The duties of a Post Acute Nurse Practitioner, APRN mid-level provider include, but are not limited to, commute to multiple post-acute care facilities every day, making rounds, communicating to patients, facility staff members, providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients.


PRIMARY FUNCTIONS

  1. Examine patients and record history
  2. Adhere to proper charting protocols
  3. Discuss with Physician and other health care professionals to prepare comprehensive plan
  4. Order or execute tests or images as needed
  5. Submit health care plan and goals of individual patients for periodic review by Physician
  6. Analyze results of tests, diagnose and report to patient
  7. Administer or prescribe treatment and medications
  8. Promote health by advising patient on diet, hygiene and prevention of disease
  9. Inoculate and vaccinate as required
  10. Refer to specialist as required for patient's well-being
  11. Report deaths and contagious diseases to governmental authorities
  12. Other responsibilities as assigned

EDUCATION AND EXPERIENCE

  1. Must have and maintain current State of Florida license in good standing
  2. Current National Board Certification
  3. Current DEA
  4. Post Acute experience
  5. 1 year Post Acute experience preferred

KNOWLEDGE, SKILLS, AND ABILITIES


  1. Effective written, verbal and electronic communication skills
  2. Show initiative, creativity and problem-solving ability
  3. Ability to collaborate with colleagues to provide quality healthcare

ADDITIONAL QUALIFICATIONS

  1. Prior experience using an electronic medical record system
  2. Bilingual preferred (English/Spanish)

RELATIONSHIP REPORTING

Reports to the Supervisory Physician / Post-Acute Executive Director

PHYSICAL REQUIREMENTS

  1. Ability to communicate effectively with patients
  2. Ability to collaborate with colleagues as necessary to provide quality health care
  3. Ability to use a computer, printer and scanner
  4. Ability to sit, stand or view a computer screen for extended periods of time
  5. Ability to perform repetitive hand and wrist motions for extended periods of time
  6. Ability to use hands and fingers to handle or feel
  7. Ability to reach with hands and arms


Compensation details: 90000-120000 Yearly Salary
