At Precision Health, we believe that exceptional healthcare begins with exceptional people. We are seeking a dedicated and compassionate Registered Nurse to join our team as a health coordinator in the preventive care space.
In the role of health coordinator, you will be responsible for providing patient care via in-office, in-home and telehealth/virtual in the Idaho Falls area.
Job Responsibilities:
Enroll patients into a comprehensive preventive health program.
Work closely with clinical staff to create individualized preventive care plans for patients.
Conduct Initial Patient Visits per Precision Health protocol in a clinic setting and occasionally a patient’s home.
Educate patients on Remote Patient Monitoring (RPM), Precision Health, and other telehealth programs.
Manage patient data across multiple systems including EHR, patient portals and other compliant data storage systems.
Monitor patient vital signs, such as pulse rate, temperature, blood pressure, and weight, and record information in patient records, creating customized care plans for them.
Engage with patients by phone, text, and video to: Remind them to record their vitals; Respond to alerts related to symptoms or physiologic data; Respond to minor technical troubleshooting and provide ongoing chronic care management.
Review the vitals, communication of results, and patient concerns to Precision Health and Primary Care Physicians as appropriate.
Assist with device ordering, managing patients, ensuring patient compliance, and generating reports to ensure the practice is reimbursed in a timely manner.
Maintain call logs for patients.

Proven experience as a registered nurse
Excellent knowledge of nursing care methods and procedures
