Job Description

Position: Team Lead, Community Nursing Services (CNS)

Location: Living in Central or Southern Utah, traveling to Price, Moab, and St. George. Travel expenses are paid for.

Reports To: Director of Nursing

Job Summary: The Team Lead is responsible for enhancing the skills and knowledge of new and existing field staff through educational programs, ensuring compliance with regulatory standards, and improving patient care quality.

Qualifications:

  • Preferred: Bachelor of Science in Nursing
  • Required: Minimum of two years’ experience in home health care or equivalent combination of education and experience
  • Strong communication, clinical nursing, organizational, and teaching skills
  • Proficient in computer use and word processing

Responsibilities:

  • Onboard new employees, including conducting shadowing, teaching charting practices, and facilitating skill assessments
  • Conduct monthly aide inservices and ensure compliance with training hours
  • Perform shared patient visits for evaluation and training purposes
  • Conduct chart audits, identify improvement areas, and provide individual clinician feedback
  • Develop and deliver educational programs to meet regulatory requirements and address identified needs
  • Maintain up-to-date knowledge of Medicare, Utah State, and federal regulations affecting home health and hospice care
  • Ensure education and regulatory compliance across branches
  • Participate in QAPI meetings, contribute to PIPS, and cover Clinical Care Coordinator duties as needed
  • Share in the administrator on-call rotation
  • Commit to excellent customer service for patients, families, physicians, and internal team members

Location & Travel: This candidate will need to live close to one of our offices in Price, Moab or St George. This person will be in working in the office 3 days a week and at home or on the road the other two. Quarterly or as needed they will travel to the other offices in Central and Southern Utah. TRAVEL IS PAID FOR.
