Registered Nurse

Reports to Director of Nursing
Supervises: Direct Support Professionals in Provision of Delegated
Salary $69,000 - $79,000 starting salary

ESSENTIAL FUNCTIONS:
1. Visual, auditory, and motor skills necessary to maintain safety standards within the facility, i.e., hear fire alarm, see an emergency situation occurring, supervise and assist in evacuation from all areas of the facility.
2. Capacity to assist physically challenged program participants in ambulation and transfer- to bed, wheelchair-, bathtub, etc., within the realm of ability of the staff member-sex and physical strength, and also the ability of strength for the participant to assist.
3. Meet qualifications for driver certification.

RESPONSIBILITIES: include, but are not limited to the following:
1. Evaluates the nursing needs of each individual receiving services. Participates in pre-admission medical evaluations for individuals applying for service; and, completes a comprehensive assessment for each newly admitted individual in need of nursing services.
2. Acts in the capacity of primary liaison with other clinicians and medical practitioners, regarding the health care needs of individuals receiving services.
3. Performs and documents required weekly face-to-face assessments for individuals receiving on-site services and monthly face-to-face assessments for those receiving off-site services, as well as annual assessments for those individuals.
4. Determines which nursing tasks and activities can be delegated to direct support staff; and delegates and supervises direct support staff in the performance of those nursing tasks and activities.
5. Provides direct support staff with supervision in the administration of prescribed medication-sand monitors effecfsof medication. Mafntains all medicationrecords and performs a formal monthly review of all medication documentation, medication administration certification and recertification of staff.
6. Provides on-going and emergency medical care and follow-up to individuals receiving services, as deemed necessary and appropriate. Accompanies individuals to medical appointments and provides medical authorities with current relevant health information. Completes necessary paperwork for dental consents and court orders for medical procedures.
7. Ensures prompt and proper medical attention to individuals receiving services for routine and special medical needs.
8. Ensures that accurate and complete records of medical and nursing services are entered into each individual's file.
9. Provides formal and informal training to direct support staff; including, but not limited to the following topics: basic health care, signs and symptoms of diseases, infection control and the performance of all delegated nursing tasks or activities. Assists in orientating newly hired staff to the health protocols of the agency.
10. Performs other related duties as may be requested by the Director.

QUALIFICATIONS:
1. EDUCATION: RN licensed to practice in New York State.
2. EXPERIENCE: One or two years experience in delivery of community based health services to the developmentally disabled. The quality of care and life in the residence should be priority to the RN.
